Credential File

Students may use the following blank reference forms when requesting a letter of reference from a professor, past employer, or supervising teacher. Please read the descriptions below to see the difference between the three forms. The completed reference form should be returned to the Career Development Center. There is no deadline and you may add a new reference at any time that you have an active file.

To get started, please download the Credential File Packet and carefully review the procedure that you will need to follow to create your Credential File in our office.

Credential File Legal Consent Form

Federal legislation and interpretations beginning with the Family Educational Rights and Privacy Act (FERPA) require colleges and universities to obtain written authorization from students and alumni before credentials can be sent to prospective employers or graduate schools.

Letter of Reference

This form is designed for the author to write a narrative letter about your abilities. Letters should be professionally prepared and attractive to the reader.

Student Teaching Reference Form

This form is designed for Education majors involved in student teaching (Block II or III). It should only be given to cooperating and supervising teachers from one's student teaching experience.

Reference Form

This form is suitable for any major and can be used when the author of the letter may not know you well enough to write a narrative letter.

In addition to the pre-printed forms available from our office, students may also solicit references from individuals on the letterhead of the person or their organization. (Note: Unless otherwise stated, references of this kind are non-confidential).

Student Employment Services

Student Employment Services Registration Form

Current students who are looking for jobs on-campus or off-campus are required to complete the Student Employment Registration form once per academic year prior to using student employment services.

Turn in the completed form to the Career Development Center to have access to the part-time and seasonal job postings maintained by the office. This is a free service available to currently enrolled SUNY Plattsburgh students.

4 Year Career Plan

This form is a "road map" for students and their families who want to know how to make the most of the college years. It contains practical career tips for students in their Freshman, Sophomore, Junior, and Senior year.
