In some cases, the Admissions Office may request your mid-term grades before reviewing your application for admission. If your college produces mid-term grades, simply print them out and staple them to this form. If not, please have your professors list your grades and sign their names on the form. Upon receipt, your application will be reviewed for admission.
If you have applied through the SUNY Application, you may be asked to submit the Transfer Supplemental Application online in your MyPlattsburgh account. You may be asked to submit "Gap Information" (if there are any chronological gaps in your academic history), or a list of courses in which you're currently enrolled if they're not already on your transcript. Within the Transfer Supplemental Application, you can also complete the transfer essay. If the supplemental is requested, you will be sent an e-mail with a link to the online Transfer Supplemental Application once you have applied and your application has been processed (which typically takes about 10 business days).
If you are not asked to submit the Transfer Supplemental Application, but wish to do so, please contact Admissions at 518-564-2040 or email@example.com, and we will update your MyPlattsburgh account accordingly.
Please note: If you have applied using the Common Application, you do not need to submit the Transfer Supplemental Application.
Transfer students who are interested in participating in the Educational Opportunity Program (EOP) should review the EOP Transfer Checklist carefully as it details the documents needed to gain admission as an EOP student. EOP provides additional support for those students who are financially and academically qualified and is reserved for those students who were previously enrolled in an EOP-type program (such as EOP, HEOP, SEEK, or College Discovery).
Transfer students who are interested in participating in the Educational Opportunity Program (EOP) must complete the transfer verification form. This form must be completed by the appropriate official(s) at the student's first college attended.
Transfer students who are applying to the Social Work program are required to submit a specific personal essay and letter of reference to be reviewed for admission. In addition to academic criteria, admission is also based upon the applicant’s suitability for a career in Social Work.
The SUNY Student Transfer Appeal Process is designed for any student in a SUNY associate degree program who does not agree with the decision of the college regarding acceptance and/or placement of credit earned elsewhere within SUNY. Completing this form will initialize the appeal process.
