Instructions

The Liability and Release Form is also a required form to participate in Odyssey. Please read through carefully and feel free to contact us if there is any need for clarification.  If Odyssey participant is 18 years or younger this form must be signed by a parent or guardian. Please complete this form and send to the below address, unaltered.
