The Eastern Adirondack Health Care Network’s mission is to improve the health status of our communities, and the access and delivery of health care in the region, through the collaboration of our members. We seek funding on behalf of our members and provide staff support to start new health improvement initiatives and build a plan for sustainability.
With better health, seniors keep their independence. With adequate supports and services, community living and nursing home diversion is possible. Our primary target population are residents, primarily age 55+, residing in Clinton, Franklin, and Essex counties in Northeastern NY, and there informal caregivers.
Our objectives: (1) Strengthen access to and provision of services and supports for our aging population, & their caregivers; (2) Increase local health system capacity by advancing workforce training and education within the region resulting in high quality care to our geriatric population; (3) Improve access to and utilization of mental health services for our geriatric population; and (4) Coordinate and collaborate with Northern NY Rural Health Networks.
