A

Aid For Part-Time Study Application

This application is for students seeking a grant award through New York State's Aid for Part-Time Study (APTS) Program.

Authorization for Release of Financial Information

Provides permission for the Student Financial Services staff to share certain financial information with authorized individuals.

Authorization to Pay Host Institution

Provides information on what documents are needed for financial aid to be approved when a student is earning credits at another institution.

Budget Adjustment Request Form

This form and supporting documentation is needed when a student experiences educational expenses above his/her cost of attendance budget. Student budgets and examples of adjustments which will be taken into consideration are provided on the form.

Consent to Obtain Credit

This form should only be submitted when a parent wishes to have their credit reviewed by the Federal Student Aid (FSA) Programs for possible eligibility for a Parent PLUS Loan. If credit is accepted, then that parent may be eligible to borrow a Parent PLUS Loan. If credit is declined, then the dependent student may be eligible to borrow an additional unsubsidized student loan, among other options.

Dependency Override Criteria Form

Provides students detail as to the criteria used by the Federal and State government's to determine a student's status as either dependent or independent. Also provides information on situations and required proof for consideration of a dependency override.

Drug Conviction Eligibility Worksheet

If you answer "yes" to question 23 of the FAFSA please complete this form and return to the Financial Aid Office.

Federal Authorization for Parent PLUS Loan

This form may be requested of a parent who has applied and been approved for a Parent PLUS loan.  Optional college fees may be paid for through a Federal PLUS Loan only when this authorization is provided.
 

Additional Financial Information Worksheet

This form provides clarification of sources of untaxed income or excludable income received in your household.

Federal Work-Study Payroll Forms

These documents need to be completed and submitted to the payroll office upon hire in the work-study program.

Financial Aid Appeal Form

This form may be needed if a student has been notified he or she has not met one or more of the minimum academic progress standards for retention of financial aid.

GI Bill / Veteran's Benefits Certification Request Form 

If you are a veteran or dependent of a veteran and would like us to certify your GI Bill, please provide this completed form to the financial aid office. It is your responsibility to request enrollment certification each semester.
 

Graduate Diversity Fellowship Program Application

SUNY Plattsburgh's Graduate Diversity Fellowship Program provides full in-state tuition for up to six students per year who contribute to the diversity of the student body in their graduate program and have overcome a disadvantage or other impediment to success in higher education.  The application deadline for the 2013-2014 academic year is February 15, 2013.

Household Asset Information

This form provides clarification of the household assets in place at the time of completing your Free Application for Federal Student Aid (FAFSA).

Household Size/Number-in-College Verification

Needed for verification of the people in your household and confirmation of other household members attending college.

Loan Adjustment Request

Needed when requesting an increase or decrease to the federal loan currently in process for you. This would be applicable to the Federal Subsidized and Unsubsidized Student Loans and the Federal Parent PLUS Loan. Contact the Financial Aid Office for your eligibility before requesting a loan increase.

Self-Certification Form

Self-certification is required for applicants of alternative (private education) loans. Contact the Financial Aid Office for guidance on financial aid resources and assistance with section 2 of the self-certification. This form should be completed and submitted to your lender.
 

Signature Certification Form

Needed when the students and/or parents signature(s) are missing from the FAFSA. Only submit this form if requested by the Financial Aid Office.

Special Circumstance Consideration Form

Needed if your household has experienced special circumstances which impact your FAFSA data and/or financial need. Examples of special circumstances that will be considered are provided on the form.

Summer Financial Aid Form

A summer financial aid form must be completed by those students interested in receiving financial aid assistance during the summer semester. This application is used to determine work-study eligibility as well as grant and loan funding.

Title IV Authorization Form

Use this form to authorize Student Accounts to apply any credit from Title IV funds in excess of tuition, mandatory fees, room, and meals to pay the remaining balance on your bill.

Verification of Child Support Paid

Students who have been selected on their FAFSA for a review of child support paid by the household are requested to submit this worksheet.

Verification of Dependent Student

Dependent students who have been selected for verification on their FAFSA are requested to submit the verification worksheet. Verification of prior year income is also required for both the student and custodial parents. Only submit this form if requested by the Financial Aid Office.

Verification of Identity and Statement of Educational Purpose

Provides proof of your identity and certification of educational purpose of your federal student financial assistance.

Verification of Independent Student

Independent students who have been selected for verification on their FAFSA are requested to submit the verification worksheet. Verification of prior year income is also required for both the student and student's spouse if applicable. Only submit this form if requested by the Financial Aid Office.

Students who have been selected on their FAFSA for a review of received SNAP benefits are requested to submit this worksheet.
